DHI Simplified Water Transfer PHA / PSSR

Field-ready PHA and pre-startup checklist for smaller water transfer, pump/meter rental, pond skimming, AST/tank transfer, layflat, hard-line piping, valves, bypasses, and simple treatment jobs.

Use note: This is intentionally simpler than the full PHA Node Tree Builder. Use it for smaller jobs where the configuration is mostly source → pump → meter/line/valves → receiving tank or pond, with optional skimmer, bypass, AST, frac tank, or DAF. It does not replace a formally facilitated PHA for complex, high-risk, high-pressure, chemical, H2S, or permit-critical operations.

2. Simplified Flow / Equipment List

Add only the major items needed to understand routing and risk: source, skimmer/intake, pump, meter, layflat or hard-line, valves/manifolds, bypasses, ASTs, frac tanks, ponds, DAF, and final discharge. Keep this high-level enough for a field PHA but specific enough to verify the valve lineup before startup.

3. Field Flow View

This is a simple routing view. It is not a full P&ID. Use it to confirm that each item has a predecessor/source and a clear outgoing destination or purpose.

4. Connection / Lineup Register

This is the field audit list for what each item receives and where it sends flow. It should match the physical valve lineup, labels, and startup procedure.

5. Simplified PHA Risk Assessment

Severity: 1 minor nuisance, 2 minor spill/equipment issue, 3 reportable spill/injury/equipment damage, 4 major spill/serious injury/business interruption, 5 life-threatening/catastrophic/environmental major event.
Likelihood: 1 unlikely with controls, 2 possible but uncommon, 3 credible during field work, 4 likely if not controlled, 5 expected/repeated exposure.

6. PSSR / Startup Punch List

Generate this after the flow and PHA rows are complete. Close required-before-startup items before mobilizing or starting flow.

Control philosophy: For small transfer jobs, do not overcomplicate the PHA, but do not rely only on someone “watching it” for credible overflow, deadhead, high pressure, or wrong-destination scenarios. Use physical labeling, valve lineup checks, pressure gauges, bypasses/relief paths, level controls, high-level alarms, pump shutdowns, containment, line walks, and clear stop-work criteria where practical.
